Treatment of CMV diarrhea primarily involves antiviral medications such as ganciclovir, valganciclovir, or foscarnet. These drugs help suppress viral replication, reduce inflammation, and promote healing of the gastrointestinal mucosa. Alongside antiviral therapy, supportive care plays a vital role. Patients are encouraged to maintain adequate hydration, manage symptoms with appropriate medications, and address nutritional needs. In some cases, reducing immunosuppressive therapy may be considered, but this must be carefully balanced against the risk of organ rejection or disease progression.
Prevention strategies include careful management of immunosuppressive therapies, regular monitoring of at-risk patients, and good hygiene practices to reduce initial infection risk. For transplant recipients or people with HIV, prophylactic antiviral medications may be prescribed to prevent CMV reactivation, thereby decreasing the likelihood of complications like diarrhea.
