The initial step in diagnosing trigeminal neuralgia involves a thorough clinical history. Patients typically report sudden, severe, electric-shock-like episodes of facial pain that can last from a few seconds to a couple of minutes. These episodes often occur in clusters and may be triggered by everyday activities such as chewing, speaking, brushing teeth, or even touching the face. Understanding the pattern, location, and triggers of the pain is crucial, as trigeminal neuralgia usually affects specific branches of the nerve—most commonly the maxillary (second branch) or mandibular (third branch).

Physical examination, although often unremarkable between episodes, remains a vital component. A neurologist will assess facial sensation, muscle strength, and reflexes to rule out other causes of facial pain such as nerve injury or tumors. Special attention is given to testing the trigeminal nerve’s sensory distribution by gently stimulating the face with light touch or pinprick to identify any abnormal responses or areas of altered sensation.

Imaging studies play a pivotal role in confirming the diagnosis and ruling out secondary causes. Magnetic resonance imaging (MRI), especially with high-resolution techniques like MRI with contrast or magnetic resonance angiography (MRA), helps visualize the trigeminal nerve and surrounding vascular structures. In many cases, trigeminal neuralgia is caused by vascular compression of the nerve root entry zone, which can be identified through imaging. If a tumor, multiple sclerosis lesions, or other structural abnormalities are suspected, further imaging or investigations may be warranted.

Electrophysiological tests are not routinely necessary but can assist in complex or ambiguous cases. Techniques such as trigeminal reflex testing or nerve conduction studies may demonstrate abnormal responses consistent with nerve hyperexcitability. These tests support the diagnosis but are typically supplementary to clinical assessment and imaging.

Laboratory tests are generally not diagnostic but may be employed to exclude other conditions like infections or systemic diseases that could mimic trigeminal neuralgia. For example, blood tests may help rule out Lyme disease, diabetes, or autoimmune disorders.

In summary, diagnosing trigeminal neuralgia involves a comprehensive approach that combines detailed patient history, careful physical examination, and targeted imaging. Recognizing the characteristic pain pattern and triggers often provides the most significant clues, while imaging confirms the presence of neurovascular compression or other structural causes. An accurate diagnosis is vital for guiding appropriate treatment options, which may include medications, nerve blocks, or surgical interventions.
