This immune response is believed to involve the production of specific antibodies called anti-neutrophil cytoplasmic antibodies (ANCAs), which are often present in GPA patients. Patients may experience persistent nasal congestion, sinusitis, nosebleeds, or ulcers in the nose and sinuses. As the disease progresses, it can lead to more severe complications such as lung nodules, cough, shortness of breath, and hemoptysis (coughing up blood). When the kidneys are affected, symptoms might include blood in the urine, proteinuria, or signs of kidney failure. Other organs that can be involved include the skin, eyes, and nervous system, leading to rashes, eye inflammation, or neurological symptoms.

Diagnosing GPA involves a combination of clinical evaluation, laboratory tests, imaging studies, and biopsy procedures. Blood tests are used to detect ANCAs and assess organ function, especially kidney function. Imaging studies like chest X-rays or CT scans can reveal lung involvement. A definitive diagnosis often requires a biopsy of affected tissue, which shows characteristic granulomatous inflammation and vasculitis.

Treatment for GPA has advanced significantly over recent decades. The primary goal is to induce remission by suppressing the abnormal immune response. This is typically achieved with a combination of corticosteroids, such as prednisone, and immunosuppressive medications, including cyclophosphamide or rituximab. Once remission is achieved, maintenance therapy with less potent immunosuppressants like azathioprine or methotrexate is often used to prevent relapse. In some cases, biologic agents targeting specific immune pathways are employed, especially for patients with refractory disease.

While GPA is a serious condition, early diagnosis and aggressive treatment can improve outcomes and significantly reduce the risk of long-term organ damage. Patients require ongoing monitoring for potential relapses and side effects of therapy. With proper management, many individuals can lead active, productive lives despite the challenges posed by this autoimmune disease.
