Takayasu arteritis is a rare, chronic inflammatory disease that primarily affects large blood vessels, especially the aorta and its main branches. It is often referred to as a “pulseless disease” because it can cause narrowing or blockage of arteries, leading to symptoms such as fatigue, limb weakness, or even organ ischemia. The cornerstone of treatment for Takayasu arteritis is immunosuppressive therapy. Corticosteroids, such as prednisone, are typically the first line of defense. However, their long-term use is associated with significant side effects—including osteoporosis, weight gain, hypertension, and increased infection risk—necessitating careful management and monitoring. One of the most commonly used steroid-sparing medications is methotrexate. It helps maintain remission and reduces the doses of steroids needed. Other immunosuppressants like azathioprine or mycophenolate mofetil may be employed if patients do not respond adequately or experience adverse effects. These drugs work by suppressing the immune response that leads to vascular inflammation, thereby preventing further vessel damage.

In cases where medical therapy alone is insufficient—particularly in patients with critical vessel stenosis or aneurysms—interventional procedures become necessary. These include angioplasty and stenting, which can reopen narrowed arteries and restore blood flow. Surgical options, such as bypass grafting or vascular reconstruction, might be considered in severe or refractory cases. However, these interventions carry risks and are typically reserved for specific situations where medical management does not suffice.

Biologic therapies represent an emerging frontier in Takayasu arteritis treatment. Drugs like tocilizumab, an interleukin-6 receptor inhibitor, have shown promise in controlling disease activity, especially for patients who are refractory to conventional immunosuppressants. These targeted therapies aim to modulate specific pathways involved in the inflammatory process, offering hope for better disease control with fewer side effects.

Monitoring disease activity is crucial for optimal management. Regular imaging studies—such as magnetic resonance angiography (MRA) or positron emission tomography (PET)—are used to assess vessel inflammation and detect complications early. Adjustments in therapy are often guided by clinical symptoms, laboratory markers of inflammation, and imaging findings.

Overall, treating Takayasu arteritis requires a personalized approach that balances controlling inflammation, preventing vascular damage, and minimizing medication side effects. Close collaboration among rheumatologists, vascular surgeons, and other specialists is essential to tailor treatment plans, improve outcomes, and enhance quality of life for affected individuals.
