Marfan syndrome is a genetic disorder that affects the body’s connective tissue, which provides support and structure to various organs and systems. Because of its complexity and variability, diagnosing and managing Marfan syndrome requires a comprehensive and multidisciplinary approach. The diagnosis process typically involves a combination of clinical evaluation, family history assessment, and genetic testing. Once diagnosed, treatment focuses on preventing complications, improving quality of life, and managing symptoms.

The initial step in diagnosing Marfan syndrome is a thorough clinical examination by a healthcare professional experienced in connective tissue disorders. The clinician assesses physical features such as tall stature, long limbs, fingers, and toes (arachnodactyly), chest deformities, scoliosis, and eye abnormalities like lens dislocation. Additionally, cardiovascular evaluation is crucial because many patients develop aortic dilation or aneurysm, which can lead to life-threatening dissection or rupture. Eye examinations, including slit-lamp assessments, help identify lens dislocation or other ocular issues.

Family history plays a vital role, as Marfan syndrome is inherited in an autosomal dominant manner. A positive family history of similar features or related complications increases suspicion and guides further testing. Genetic testing for mutations in the FBN1 gene, which encodes the fibrillin-1 protein, can confirm the diagnosis. However, because not all mutations are detectable, a combination of clinical criteria and genetic analysis is often necessary.

The revised Ghent nosology is widely used as a diagnostic criteria tool. It integrates clinical features and genetic information to categorize the likelihood of Marfan syndrome. A diagnosis is established when specific major criteria are met across multiple organ systems or when a combination of features and genetic findings align.

Treatment protocols are tailored to individual patient needs, with a primary focus on preventing serious complications. Regular surveillance of the aorta via echocardiography, MRI, or CT scans is essential to monitor for dilation. Pharmacological management often involves the use of beta-blockers or angiotensin receptor blockers, which have been shown to slow aortic dilation. Lifestyle modifications, such as avoiding strenuous physical activities that increase blood pressure, are recommended to reduce stress on the aorta.

Surgical intervention may become necessary if the aortic root reaches a critical size or shows rapid growth, to prevent rupture. Procedures like aortic root replacement are performed by specialized cardiovascular surgeons. Eye issues such as lens dislocation are managed with glasses or surgical correction when needed, and regular ophthalmologic assessments are advised.

In addition to addressing cardiovascular and ocular issues, a multidisciplinary team approach—including cardiologists, geneticists, orthopedists, and ophthalmologists—is vital for comprehensive care. Psychosocial support and genetic counseling are also integral parts of the treatment protocol, helping patients and families understand the condition, inheritance patterns, and implications for family planning.

Overall, early diagnosis and tailored management can significantly improve outcomes for individuals with Marfan syndrome, enabling them to lead active, healthier lives while minimizing the risk of severe complications.
