Glioblastoma, also known as glioblastoma multiforme, is an aggressive form of brain cancer that poses significant treatment challenges. Due to its rapid progression and complex nature, understanding the treatment timeline and associated complications is essential for patients, caregivers, and medical professionals alike. The treatment process is typically multidisciplinary, involving surgery, radiation therapy, chemotherapy, and emerging targeted treatments, each with specific timelines and potential complications.

The first step often involves surgical intervention aimed at removing as much of the tumor as possible. This procedure, called a craniotomy, is performed under general anesthesia and requires careful planning to minimize damage to surrounding healthy brain tissue. Post-surgery, patients usually undergo histopathological examination to confirm the diagnosis and assess tumor grade. While surgical removal can alleviate symptoms and reduce tumor burden, it is rarely curative due to the infiltrative nature of glioblastomas. Complications from surgery can include infections, bleeding, neurological deficits, or edema, which require prompt management.

Following surgery, radiotherapy becomes a central component of the treatment timeline. Typically starting within 2 to 4 weeks post-surgery, radiation aims to target residual tumor cells. The standard regimen involves daily sessions over 6 weeks, delivering a total dose carefully calibrated to maximize tumor control while sparing healthy tissue. Radiation complications may include fatigue, skin irritation, hair loss, and, less commonly, cognitive changes or radiation necrosis. These side effects are monitored closely, and supportive care is provided to mitigate discomfort.

Concurrently or shortly after radiotherapy, chemotherapy with temozolomide (TMZ) is often introduced, forming part of the Stupp protocol, the current standard of care. Chemotherapy usually begins within a few weeks of radiation completion, administered daily during radiation and then on a schedule for several months. Temozolomide enhances the effectiveness of radiation therapy but also brings additional side effects, such as nausea, fatigue, immune suppression, and, rarely, blood count abnormalities. Regular blood tests are necessary to monitor for hematological toxicity.

Emerging treatments, including targeted therapies, tumor-treating fields, and immunotherapy, are being integrated into the treatment timeline based on individual patient factors and clinical trial availability. These therapies may extend the timeline and require additional monitoring and management.

Throughout this treatment journey, vigilant monitoring for complications is vital. Imaging studies, primarily MRI scans, are scheduled at regular intervals to assess tumor response and detect recurrence early. Patients may also experience neurological symptoms, seizures, or cognitive changes that necessitate supportive interventions like medications or rehabilitative therapy.

Prognosis remains cautious, given glioblastoma’s aggressive course, but advances in multimodal therapies continue to improve quality of life and extend survival for many patients. The treatment timeline is dynamic and tailored to each individual, emphasizing the importance of a comprehensive, coordinated approach that balances aggressive tumor control with quality of life considerations.
