Creutzfeldt-Jakob Disease (CJD) is a rare, degenerative neurological disorder caused by abnormal prion proteins that lead to brain tissue destruction. Its clinical features can be subtle initially but tend to progress rapidly, making early recognition crucial for diagnosis and management. The disease typically manifests in middle-aged or elderly individuals, with various forms including sporadic, hereditary, and acquired variants, each sharing overlapping clinical presentations.

The initial symptoms of CJD are often nonspecific and can include subtle cognitive changes such as memory loss, difficulty concentrating, or subtle personality shifts. As the disease advances, patients commonly develop rapidly progressive dementia characterized by profound confusion, disorientation, and impaired judgment. This cognitive decline is often accompanied by psychiatric symptoms such as anxiety, depression, or hallucinations, which may lead to misdiagnosis in the early stages.

Motor symptoms are a hallmark of Creutzfeldt-Jakob Disease and tend to emerge as the disease progresses. These include myoclonus—sudden, involuntary muscle jerks that are often stimulus-sensitive—and ataxia, which manifests as unsteady gait and coordination problems. Patients may also develop rigidity and tremors, mimicking other neurodegenerative disorders, but the rapid course distinguishes CJD from most other conditions.

A characteristic feature of CJD is the development of visual disturbances, such as blurred vision or visual field deficits, which result from cortical or subcortical involvement of the visual pathways. As the disease advances further, patients often become bedridden and lose the ability to speak or swallow, ultimately succumbing usually within a year of symptom onset. The rapid progression from initial symptoms to severe neurological decline underscores the aggressive nature of the disease.

Reflex abnormalities are also notable, with myoclonus being particularly prominent and often triggered by external stimuli like noise or light. Seizures are uncommon but can occur in some cases. Additionally, signs of akinetic mutism—where patients are awake but unresponsive—may be observed in the later stages.

From a clinical perspective, the diagnosis of CJD relies heavily on a combination of neurological examination, characteristic clinical features, and supportive investigations. These include electroencephalography (EEG), which may show periodic sharp wave complexes, and magnetic resonance imaging (MRI) demonstrating hyperintense signals in the cortical ribbon or basal ganglia. Cerebrospinal fluid (CSF) analysis can reveal elevated 14-3-3 protein levels, although these findings are not definitive. Brain biopsy or post-mortem examination remains the gold standard for confirmation.

Understanding the clinical features of Creutzfeldt-Jakob Disease is essential for early suspicion, although definitive diagnosis is complex. The rapid deterioration and distinctive neurological signs emphasize the importance of prompt recognition, primarily to differentiate CJD from other treatable neurological conditions and to provide appropriate patient counseling.
