Since there is no single definitive test for psoriatic arthritis, healthcare providers rely on a combination of clinical evaluations, imaging studies, and laboratory tests to arrive at an accurate diagnosis. Understanding these diagnostic tools helps patients and clinicians work together to distinguish psoriatic arthritis from other joint diseases and initiate appropriate treatment.

The initial step in diagnosing psoriatic arthritis involves a thorough medical history and physical examination. Patients are asked about their skin symptoms, such as psoriasis, and joint symptoms, including swelling, pain, stiffness, and reduced mobility. The pattern of joint involvement—whether it affects just a few joints or many, and whether it involves the fingers, toes, or spine—can provide vital clues. Psoriatic arthritis often affects the distal interphalangeal joints (near the fingernails), which can help differentiate it from other types of arthritis.

Laboratory tests are essential in ruling out other conditions like rheumatoid arthritis or gout, as there are no blood tests that can definitively confirm psoriatic arthritis. Erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) are common blood tests used to detect inflammation in the body. Elevated levels may support the diagnosis of an inflammatory arthritis but are not specific to psoriatic arthritis. Rheumatoid factor (RF), a blood antibody often positive in rheumatoid arthritis, is usually negative in psoriatic arthritis, helping distinguish between the two. Another important blood test is the human leukocyte antigen B27 (HLA-B27) test.
