Takayasu Arteritis is a rare, chronic inflammatory disease that primarily affects large blood vessels, notably the aorta and its major branches. Although more commonly diagnosed in young women, it can also occur in children, where its identification poses unique challenges. Recognizing and diagnosing Takayasu Arteritis early in pediatric patients is crucial, as it can prevent severe complications such as organ ischemia, hypertension, and vascular damage.

Children with Takayasu Arteritis often present with nonspecific symptoms, making diagnosis complex. Common early signs include fatigue, fever, weight loss, and malaise, which are easily mistaken for other pediatric illnesses. As the disease progresses, symptoms related to vascular involvement emerge, such as limb claudication, decreased or absent pulses in the arms or legs, hypertension, and differences in blood pressure readings between limbs. Some children may also experience dizziness, headaches, or visual disturbances due to cerebral ischemia.

A thorough clinical evaluation begins with a detailed medical history and physical examination. Clinicians look for pulse discrepancies and blood pressure differences between limbs, as well as auscultatory bruits over the carotid or subclavian arteries. The presence of systemic symptoms alongside vascular signs raises suspicion for large vessel vasculitis like Takayasu Arteritis.

Laboratory investigations serve as supportive tools but are not definitive on their own. Elevated inflammatory markers, such as erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP), are common findings that indicate ongoing inflammation. However, these markers

are nonspecific and can be elevated in various inflammatory conditions. Additional tests may include anemia assessment and blood counts. Serologic testing for infections or autoimmune markers such as antinuclear antibodies (ANA) can help exclude other causes.

Imaging studies are essential for confirming the diagnosis and assessing the extent of vascular involvement. Doppler ultrasound is a useful initial modality to evaluate large arteries for wall thickening, stenosis, or occlusion. Magnetic resonance angiography (MRA) and computed tomography angiography (CTA) provide detailed visualization of blood vessels, revealing characteristic features such as vessel wall thickening, narrowing, or aneurysms. Conventional angiography, though more invasive, remains the gold standard for definitive diagnosis and delineating vascular lesions.

In pediatric cases, the diagnosis often requires a high index of suspicion due to the disease’s subtle presentation. Combining clinical findings with laboratory and imaging results is vital for accurate identification. Early diagnosis enables prompt treatment, primarily with corticosteroids and immunosuppressive agents, to reduce inflammation and prevent irreversible vascular damage. Multidisciplinary management involving pediatric rheumatologists, radiologists, and vascular specialists is often necessary to optimize outcomes.

In summary, diagnosing Takayasu Arteritis in children involves recognizing the constellation of systemic and vascular symptoms, supporting these with laboratory findings, and confirming vascular changes through advanced imaging. Early intervention is key to improving quality of life and reducing the risk of severe complications associated with this elusive disease.
