It leads to progressive neurological decline, vision loss, seizures, and eventually, premature death. Early and accurate diagnosis is crucial, not only for managing symptoms and planning supportive care but also for families considering genetic counseling and future family planning. Several testing options are available today to identify Batten disease, each with its own advantages and specific applications.

The first and most common testing approach involves genetic testing, which detects mutations in the genes associated with Batten disease. There are multiple forms of the disease, with the most common being CLN3-related Batten disease. Genetic testing can be performed through a blood sample or saliva sample, providing a definitive diagnosis by identifying pathogenic mutations. This method is highly accurate and can confirm whether an individual carries a mutation, is affected by the disease, or is a carrier of the gene mutation without showing symptoms. For families with a known history of Batten disease, targeted genetic testing can pinpoint specific mutations, assisting in early diagnosis and risk assessment.

Another key diagnostic tool is enzyme activity testing, particularly for certain forms of Batten disease caused by enzyme deficiencies, such as juvenile and adult-onset forms linked to specific enzyme abnormalities. In these cases, a blood sample or skin biopsy can be used to measure the activity of relevant enzymes. Reduced or absent enzyme activity can confirm the diagnosis in cases where genetic testing indicates a mutation but functional confirmation is needed. This method is especially useful in distinguishing between different types of neuronal ceroid lipofuscinosis.

They are often used alongside genetic or enzyme testing to build a comprehensive picture of the disease’s progression.

Further diagnostic options include skin biopsies, which can be examined under electron microscopy to detect characteristic storage material accumulations in cells. While more invasive, skin biopsies can be instrumental when genetic or enzyme tests yield inconclusive results, particularly in atypical cases.

Advances in molecular diagnostics have also led to the development of newborn screening programs in some regions, aiming to identify Batten disease early before clinical symptoms manifest. Early detection through newborn screening can facilitate timely intervention and planning, although widespread implementation is still under consideration.

In summary, diagnosing Batten disease involves a combination of tests tailored to the specific subtype suspected. Genetic testing remains the cornerstone, providing definitive results.
