Wilson’s disease is a rare inherited disorder characterized by the accumulation of copper in the body, leading to neurological, hepatic, and psychiatric symptoms. Early diagnosis is crucial to prevent irreversible organ damage and to initiate appropriate treatment. However, diagnosing Wilson’s disease can be challenging due to its diverse clinical presentation and the overlap of symptoms with other conditions. Understanding the diagnostic approach involves a combination of clinical assessment, laboratory tests, imaging studies, and genetic analysis.

Clinicians begin by taking a detailed medical history and performing a thorough physical examination to identify characteristic signs such as tremors, rigidity, jaundice, or behavioral changes. A high index of suspicion is necessary, particularly in young patients presenting with unexplained neurological or liver symptoms. Recognizing Kayser-Fleischer rings—brownish or greenish rings around the cornea—via slit-lamp examination remains a vital clinical clue, though their absence does not exclude the diagnosis.

Laboratory evaluations are fundamental in confirming Wilson’s disease. Serum ceruloplasmin, a copper-carrying protein, is typically low in affected individuals, but this is not specific and can be influenced by other conditions. Serum copper levels can be paradoxically low or normal, so they are not solely relied upon. A more sensitive marker is the 24-hour urinary copper excretion; elevated levels (usually exceeding 100 micrograms per day) suggest abnormal copper metabolism. However, these tests can sometimes be inconclusive or affected by recent copper intake or therapy.

A crucial diagnostic tool is the detection of copper accumulation in tissues, most notably through liver biopsy. Quantitative measurement of hepatic copper content exceeding 250 micrograms per gram of dry weight strongly supports the diagnosis. Histological examination often reveals hepatic inflammation or fibrosis, providing additional clues.

Genetic testing plays an increasingly important role, especially in ambiguous cases. Identification of mutations in the ATP7B gene, responsible for copper transport, confirms the diagnosis. However, since not all mutations are known or tested, genetic analysis is often combined with biochemical and clinical assessments.

Imaging studies, such as brain MRI, can reveal characteristic changes in the basal ganglia, thalamus, or cerebellum in patients with neurological symptoms. These findings, while supportive, are not diagnostic on their own but contribute to the overall clinical picture.

In summary, diagnosing Wilson’s disease requires an integrated approach that combines clinical suspicion with laboratory findings, tissue analysis, genetic testing, and neuroimaging. Early recognition and diagnosis are pivotal in managing the disease effectively and preventing severe complications. Awareness of the characteristic signs and a systematic diagnostic workup can lead to timely intervention, significantly improving patient outcomes.
