Due to its complex presentation, diagnosing and managing TN requires a careful, systematic approach that involves clinical evaluation, diagnostic imaging, and individualized treatment protocols.

The initial step in diagnosing trigeminal neuralgia involves a detailed patient history. Clinicians will inquire about the nature, duration, frequency, and triggers of facial pain. Patients often describe episodes of stabbing or burning pain that are typically unilateral and follow a specific nerve distribution—most commonly affecting the maxillary (V2) or mandibular (V3) branches of the trigeminal nerve. Identifying any patterns—such as pain provoked by chewing, speaking, or touch—is crucial, as these are hallmark features of TN. Additionally, it’s essential to rule out other facial pain disorders like dental issues, temporomandibular joint disorders, or neuralgias caused by other cranial nerves.

Typically, in classical trigeminal neuralgia, neurological deficits are absent, but the presence of sensory loss or weakness may suggest alternative diagnoses such as multiple sclerosis or tumors.

Magnetic Resonance Imaging (MRI) is the gold standard, especially high-resolution MRI sequences like MRI with fat suppression or MR angiography. These imaging modalities help visualize vascular compression of the trigeminal nerve root entry zone—a common cause of classical TN—and detect lesions like tumors or demyelinating plaques in secondary causes. Vascular contact or compression seen on MRI strongly supports a diagnosis of classical TN.

Once diagnosis is confirmed, establishing a treatment plan depends on the severity of symptoms, patient preferences, and underlying causes. Medical management is usually the first line and involves anticonvulsant medications such as carbamazepine or oxcarbazepine. These drugs stabilize nerve activity and reduce pain episodes effectively in many patients. However, they need careful titration and monitoring for side effects like dizziness, drowsiness, or blood dyscrasias.

For patients who do not respond adequately to medication or experience intolerable side effects, surgical options are considered. Microvascular decompression (MVD) is a common surgical procedure that relieves nerve compression by repositioning or removing offending blood vessels. Other less invasive procedures include percutaneous techniques such as glycerol injection, balloon compression, or radiofrequency thermocoagulation, which aim to damage the nerve fibers to block pain signals. Stereotactic radiosurgery, like Gamma Knife, is another alternative, especially for patients who are not surgical candidates.

Effective management of trigeminal neuralgia often requires a multidisciplinary approach, involving neurologists, neurosurgeons, and pain specialists. Regular follow-up is essential to adjust treatment, monitor for side effects, and assess the efficacy of interventions. With proper diagnosis and tailored treatment, many patients can experience significant pain relief and improved quality of life.
