Genetic modifiers, which are other genes that can influence the expression of the FBN1 mutation, may affect the severity and specific features of the syndrome. This variability explains why some individuals develop severe cardiovascular issues early in life, while others have milder skeletal or ocular symptoms.

Age and gender can also influence the presentation. Typically, signs and symptoms become more apparent during adolescence or early adulthood, when rapid growth can exacerbate skeletal and cardiovascular issues. Males are often more severely affected in terms of cardiovascular complications, possibly due to differences in hormonal influences or genetic modifiers.

Environmental factors are less directly associated but can influence disease management and outcomes. For instance, high-impact physical activities and strenuous exercise may increase stress on the aorta, risking dissection or rupture in susceptible individuals. Awareness and lifestyle adjustments are therefore critical components of managing risk.

In summary, the primary risk factor for Marfan syndrome is a genetic mutation in the FBN1 gene, inherited in an autosomal dominant pattern. Family history remains a strong predictor, but spontaneous mutations also contribute significantly.
