The condition is characterized by progressive loss of coordination, muscle weakness, and other neurological symptoms that can significantly impair daily functioning and quality of life. Diagnosis of Friedreich’s ataxia begins with a comprehensive clinical evaluation. Physicians perform a detailed medical history review, focusing on neurological symptoms such as gait disturbances, balance issues, muscle weakness, and speech difficulties. Family history is also vital, as the condition follows an autosomal recessive inheritance pattern, meaning that both copies of the responsible gene must be mutated for the disease to manifest. Physical and neurological examinations are conducted to assess coordination, reflexes, muscle strength, and sensory functions. The hallmark signs include impaired gait, absent reflexes in the lower limbs, and signs of scoliosis or hypertrophic cardiomyopathy, which are common associated features. These clinical findings, however, are not definitive on their own, necessitating further laboratory testing.

Genetic testing plays a central role in confirming the diagnosis. Friedreich’s ataxia is caused by a mutation in the FXN gene, which leads to reduced production of the frataxin protein. Most cases involve an abnormal expansion of GAA trinucleotide repeats within this gene. Laboratory analysis typically involves a blood sample to analyze the number of GAA repeats. A significantly expanded repeat expansion confirms the diagnosis. Genetic testing is highly specific and sensitive, making it the gold standard for diagnosis.

In addition to genetic testing, other assessments may be employed to evaluate the extent of neurological involvement and organ function. MRI scans of the brain and spinal cord can reveal atrophy in specific regions, such as the cerebellum, which correlates with coordination issues. Cardiac evaluations, including echocardiograms, are important because Friedreich’s ataxia often involves hypertrophic cardiomyopathy, which requires monitoring and management.

While there is no cure for Friedreich’s ataxia, early diagnosis allows for better symptom management and supportive therapies. Physical therapy and occupational therapy can help maintain mobility and independence for as long as possible. Cardiac monitoring and management are essential to prevent complications. Genetic counseling also plays a critical role for affected families, providing information about inheritance patterns, carrier testing, and family planning options.
