Ehlers-Danlos Syndrome (EDS) is a group of rare hereditary connective tissue disorders characterized by a variety of symptoms primarily affecting skin, joints, and blood vessel walls. Despite being relatively uncommon, with some types affecting approximately 1 in 5,000 individuals, EDS can significantly impact quality of life due to its diverse manifestations. Diagnosing EDS can be complex because its signs often overlap with other conditions, and there is no single definitive test. Instead, a comprehensive evaluation combining clinical assessment, family history, and sometimes genetic testing is essential.

The diagnosis process begins with a detailed medical history. Physicians typically inquire about the patient’s family history of connective tissue disorders, as many forms of EDS are inherited in an autosomal dominant or recessive pattern. Individuals may report symptoms such as hyperextensible skin that stretches beyond normal limits, fragile skin that bruises easily, or abnormal scarring. Joint hypermobility—excessive flexibility of the joints—is often a hallmark feature, sometimes accompanied by joint dislocations or frequent sprains. Patients may also experience chronic pain, fatigue, or issues related to blood vessel fragility, depending on the subtype.

Physical examination plays a crucial role in diagnosis. Healthcare providers assess skin elasticity, joint mobility, and the presence of characteristic features like translucent skin or abnormal scarring. They may perform specific tests, such as the Beighton score, to quantify joint hypermobility. The Beighton score evaluates the ability to perform certain movements, with higher scores indicating greater hypermobility and supporting the suspicion of EDS.

While clinical criteria are often sufficient for diagnosing some types, particularly the hypermobile type (hEDS), laboratory tests are sometimes employed to identify specific genetic mutations. Genetic testing can confirm the diagnosis in certain subtypes, such as the vascular type (vEDS), which involves mutations in the COL3A1 gene. However, genetic testing may not always detect mutations in all cases, especially in the hypermobile type, which currently has no definitive genetic marker. Therefore, the diagnosis of EDS often remains a clinical one, based on a constellation of signs and symptoms.

Imaging studies, such as ultrasounds or MRIs, might be used to evaluate joint stability or vascular abnormalities if indicated. Multidisciplinary management is crucial, as EDS can affect multiple organ systems. Rheumatologists, geneticists, cardiologists, and physical therapists often collaborate to develop personalized care plans that address symptoms, prevent complications, and improve quality of life.
