A deviated nasal septum occurs when the thin wall (nasal septum) between the nasal passages is displaced to one side. This condition is common, affecting a significant portion of the population, and can lead to symptoms such as nasal congestion, difficulty breathing through one or both nostrils, frequent sinus infections, nosebleeds, and sometimes facial pain. Diagnosing a deviated septum accurately is crucial for determining appropriate treatment, and imaging techniques, particularly X-rays, play an important role in this process.

Traditionally, nasal septum deviations were diagnosed primarily through physical examination by an ENT specialist, who would use a nasal speculum and a headlight to inspect the nasal cavity. However, physical examination alone may not reveal the full extent of the deviation, especially if the deviation is posterior or complex. Therefore, imaging studies, including X-rays, are often employed to obtain a clearer picture of the nasal anatomy.

The standard X-ray that has been used historically is called an occipito-mental (Waters) view or Caldwell view, which provides an anterior-posterior image of the nasal structures. These images can help identify the presence of septal deviations, nasal bone fractures, or other structural abnormalities. However, traditional X-ray imaging has limitations because the complex three-dimensional shape of the nasal cavity cannot be fully appreciated in two-dimensional images. Overlapping structures and variations in positioning may obscure the assessment.

Computed tomography (CT) scans are now considered the gold standard for evaluating a deviated nasal septum. Unlike traditional X-rays, CT imaging provides detailed cross-sectional images of the nasal and sinus anatomy. This allows for precise localization and measurement of the deviation, as well as assessment of any associated abnormalities such as sinusitis, turbinate hypertrophy, or concha bullosa. CT scans also help in surgical planning, giving surgeons comprehensive insights into the nasal structure.

When performing a nasal X-ray or CT scan, the patient is typically positioned lying down or sitting upright, with the head stabilized to prevent movement. The radiologist or technician ensures proper alignment to optimize image quality. During the imaging process, the patient is asked to breathe normally, and sometimes to hold their breath briefly, to minimize motion artifacts.

While X-ray imaging is valuable, it is important to understand its limitations. X-ray images are less effective in highlighting soft tissue details compared to CT scans. Therefore, in cases where soft tissue assessment or complex structural evaluation is necessary, a CT scan is preferred. Additionally, radiation exposure should be minimized, especially in pediatric patients, and imaging should be justified based on clinical indications.

While conventional X-rays can provide initial insights, advanced imaging like CT scans offers a more comprehensive assessment, guiding effective treatment plans.
