Accurate and timely diagnosis is essential to initiate appropriate interventions and minimize brain damage.

The initial suspicion of vasospasm typically arises in the clinical setting when a patient shows new or worsening neurological deficits days after the initial hemorrhagic event. However, clinical signs alone can be unreliable, especially in sedated or comatose patients, making objective diagnostic tools indispensable.

Imaging techniques are at the forefront of vasospasm detection. Transcranial Doppler (TCD) ultrasonography is widely used due to its non-invasive nature, ease of repeatability, and ability to monitor blood flow velocities in basal cerebral arteries. Elevated flow velocities, particularly in the middle cerebral artery (MCA), are indicative of vasospasm. However, TCD has limitations, as it is operator-dependent and may have reduced sensitivity in certain patient populations or in detecting distal vessel vasospasm.

Computed tomography angiography (CTA) and magnetic resonance angiography (MRA) offer more detailed visualization of cerebral arteries. CTA, in particular, has become a valuable tool for detecting vasospasm, revealing vessel narrowing with high resolution. It can be performed rapidly and is more accessible in many centers. MRI/MRA provides excellent soft tissue contrast and can assess perfusion, but it is less suitable for emergency settings due to longer scan times and contraindications in some patients.

Digital subtraction angiography (DSA) remains the gold standard in vasospasm diagnosis. It provides detailed and dynamic visualization of cerebral vasculature, allowing precise assessment of vessel caliber and the extent of narrowing. DSA also offers the advantage of facilitating concurrent therapeutic interventions, such as angioplasty or intra-arterial vasodilator administration. Despite its invasiveness and associated risks, DSA’s high sensitivity and specificity make it indispensable when non-invasive methods yield inconclusive results or when definitive diagnosis is necessary.

In addition to imaging, perfusion studies, such as CT perfusion (CTP) and perfusion MRI, can identify regions of compromised blood flow, helping differentiate vasospasm-related ischemia from other causes. These assessments enhance clinical decision-making by illustrating functional impairment, not just vessel constriction.

Laboratory tests are generally supportive rather than diagnostic for vasospasm. Cerebrospinal fluid analysis may reveal xanthochromia, confirming hemorrhage, but does not directly indicate vasospasm. Instead, the focus remains on clinical assessment and imaging findings to guide diagnosis.

In conclusion, detecting cerebral vasospasm relies on a combination of clinical vigilance and advanced imaging techniques. Early recognition through tools like TCD, CTA, MRA, and DSA is vital in initiating timely treatment strategies, such as hypertensive therapy, endovascular interventions, or pharmacological vasodilation, to prevent ischemic injury and improve patient outcomes.
