One of the primary physical examination techniques is the Tinel’s sign test. During this exam, a healthcare provider taps gently over the ulnar nerve at the elbow. If the tapping elicits tingling or “pins and needles” sensations in the ring and little fingers, it suggests nerve irritation or entrapment. While simple, this test can be quite revealing, especially when combined with other assessments.

Another common test is the elbow flexion test, also known as the Phalen’s test for the ulnar nerve. The patient is asked to fully flex the elbow and hold this position for about 60 seconds. If symptoms such as numbness or tingling intensify during this period, it indicates ulnar nerve compression at the cubital tunnel. This maneuver increases pressure on the nerve, reproducing symptoms in many cases.

The nerve conduction study (NCS) is a more sophisticated diagnostic tool. It measures how quickly electrical impulses travel along the ulnar nerve. During this test, small electrodes are placed on the skin over the nerve, and a mild electrical impulse is delivered. Slower conduction velocities suggest nerve compression or damage. NCS provides objective data that can confirm the diagnosis and quantify the severity of nerve impairment.

Electromyography (EMG) is often performed alongside NCS to assess the electrical activity of muscles supplied by the ulnar nerve. This helps determine whether the nerve compression has caused muscle weakness or atrophy, which is critical for planning treatment.

Imaging studies, such as ultrasound or MRI, are also valuable. Ultrasound can visualize the nerve and surrounding structures, identifying potential causes of compression, such as cysts or structural abnormalities. MRI provides detailed images of soft tissues, helping to assess nerve swelling or damage and rule out other pathologies.

In some cases, provocative tests like the Froment’s sign are used to assess functional impairment. When a patient attempts to grip a piece of paper between the thumb and index finger, weakness in this action can indicate ulnar nerve dysfunction.

Understanding these tests is crucial for healthcare providers to accurately diagnose Cubital Tunnel Syndrome. The combination of physical examinations, nerve conduction studies, EMG, and imaging provides a comprehensive assessment, guiding effective treatment strategies whether conservative management or surgical intervention.
