The journey toward diagnosis begins with a thorough medical history and physical examination. Healthcare providers inquire about symptoms such as headaches, vision problems, hormonal imbalances, growth delays in children, or unexplained weight changes. These signs often prompt further investigations, especially if they persist or worsen.

Following the initial assessment, the next crucial step is neuroimaging. Magnetic Resonance Imaging (MRI) is the preferred modality because of its high-resolution visualization of brain structures. An MRI scan provides detailed images of the brain, highlighting the presence, size, and exact location of the tumor. It helps distinguish craniopharyngiomas from other sellar or suprasellar masses, such as pituitary adenomas or Rathke’s cleft cysts. Sometimes, a computed tomography (CT) scan may be used, especially if there is suspicion of calcifications within the tumor, which are common in craniopharyngiomas. CT scans can also evaluate the tumor’s effect on adjacent bone structures.

Laboratory tests are integral to the diagnostic process, especially since craniopharyngiomas can affect hormonal production. Blood tests assess pituitary hormone levels, including cortisol, thyroid hormones, growth hormone, and sex hormones. Abnormal levels can suggest pituitary gland dysfunction, reinforcing suspicion of a tumor in that area. In some cases, cerebrospinal fluid (CSF) analysis may be performed if there is suspicion of tumor spread or other complications, although this is less common.

Once imaging confirms the presence of a tumor consistent with craniopharyngioma, biopsy or surgical intervention is often necessary for definitive diagnosis. This may be performed through minimally invasive techniques or during planned surgical removal. Histopathological examination of tissue samples provides

confirmation by revealing characteristic features, such as cystic components, calcifications, and specific cellular patterns.

The diagnostic process does not end with imaging and pathology. Given the tumor’s proximity to critical neurovascular structures, a multidisciplinary team—including neurosurgeons, endocrinologists, radiologists, and oncologists—collaborates to plan the optimal treatment approach. Additional assessments, such as visual field testing, are also performed because craniopharyngiomas often cause visual disturbances due to their location.

In summary, diagnosing craniopharyngioma involves a combination of clinical suspicion based on symptoms, detailed neurological and hormonal evaluations, advanced imaging studies, and histological confirmation. Early diagnosis is essential for managing the tumor effectively and minimizing long-term complications, especially those related to hormonal imbalances and vision loss.
