The cytological assessment provides essential clues to distinguish craniopharyngioma from other sellar and suprasellar lesions, such as Rathke’s cleft cysts, hypothalamic gliomas, or pituitary adenomas.

The hallmark cytological features of craniopharyngioma include the presence of stratified squamous epithelial cells, often arranged in clusters or sheets. These cells may display keratinization, with some forming keratin pearls, a characteristic feature of squamous differentiation. Additionally, the aspirate frequently reveals wet keratin, a coagulum of keratin debris mixed with granular material, which is a distinctive finding. The background may contain cystic fluid rich in cholesterol crystals, foamy macrophages, and inflammatory cells, reflecting the cystic nature of many tumors.

One of the diagnostic challenges in cytology is differentiating craniopharyngioma from other cystic lesions of the sellar region, especially when sampling yields limited cellular material. The identification of characteristic keratinized squamous cells and keratin debris is crucial. Immunocytochemistry can further aid diagnosis, with tumor cells often expressing cytokeratins, while the cystic fluid may contain cholesterol clefts and inflammatory markers.

Understanding the cytological features is essential not only for diagnosis but also for planning appropriate treatment. Surgery remains the mainstay, aiming to remove or reduce the tumor while preserving surrounding structures. Accurate cytological diagnosis can guide surgeons and neurologists in planning minimally invasive approaches, especially in cases where biopsy is challenging.

Advances in imaging, such as MRI, complement cytology by providing detailed anatomical context. However, cytological analysis offers cellular-level insights that imaging cannot provide alone. Recognizing the typical cytological features of craniopharyngioma enhances diagnostic confidence and helps avoid misdiagnosis with other sellar lesions.
