Anatomically, the cranial base is divided into three main regions: the anterior, middle, and posterior cranial fossae. The anterior cranial fossa houses the frontal lobes and the cribriform plate of the ethmoid bone, through which the olfactory nerves pass. The middle cranial fossa contains the temporal lobes, the sella turcica housing the pituitary gland, and the openings for cranial nerves III to VIII. The posterior cranial fossa accommodates the cerebellum, brainstem, and foramen magnum, through which the spinal cord exits the skull. These regions are interconnected by foramina and canals that transmit nerves and blood vessels, making the cranial base a conduit for critical neurovascular pathways.

The complex anatomy of the cranial base makes it particularly susceptible to a variety of disorders, ranging from congenital abnormalities to acquired pathologies. Congenital conditions like craniosynostosis involve premature fusion of sutures, potentially affecting the shape and development of the skull base. Other congenital anomalies include developmental cysts and syndromes such as CHARGE or Treacher Collins, which involve malformations of the craniofacial structures.

Tumors are among the most significant acquired disorders affecting the cranial base. These include meningiomas, schwannomas, chordomas, and chondrosarcomas. Due to the proximity of vital structures, these tumors often pose diagnostic and surgical challenges. Advanced imaging techniques such as MRI and CT scans are crucial for

accurate assessment, guiding surgical planning to minimize damage to surrounding neurovascular elements.

Trauma is another common cause of cranial base disorders, often resulting in fractures that can damage nerves, blood vessels, or brain tissue. Such injuries may lead to cerebrospinal fluid leaks, cranial nerve deficits, or intracranial hemorrhages. Infections like sinusitis or osteomyelitis can also extend to the skull base, causing further complications.

Management of cranial base disorders requires a multidisciplinary approach. Surgical intervention is often necessary, especially for tumor removal or repair of traumatic injuries. Innovations in surgical techniques, including minimally invasive endoscopic approaches, have significantly improved patient outcomes by reducing morbidity associated with traditional open surgeries. Additionally, radiotherapy and chemotherapy are employed in certain tumor cases, often as adjuncts or alternatives to surgery.
