Complications of Moyamoya Disease Moyamoya disease is a rare, progressive cerebrovascular disorder characterized by the narrowing and eventual occlusion of the internal carotid arteries and their main branches at the base of the brain. As the primary arteries become constricted, the brain compensates by developing a network of tiny, fragile collateral vessels that resemble a “puff of smoke” on imaging—hence the name “moyamoya,” which means “hazy” or “puff of smoke” in Japanese. While this compensatory process can temporarily maintain cerebral blood flow, it predisposes patients to various serious complications, which can significantly impact quality of life and require prompt management.

One of the most common and severe complications of Moyamoya disease is ischemic stroke. Due to the progressive narrowing of primary arteries, some regions of the brain may experience insufficient blood supply, leading to ischemia. This can manifest as sudden weakness, numbness, speech difficulties, or visual disturbances, often affecting one side of the body or face. In children, ischemic strokes are particularly prevalent, and recurrent episodes can result in cumulative neurological deficits. The risk of ischemic events underscores the importance of early diagnosis and intervention to restore or improve cerebral perfusion.

Hemorrhagic stroke is another major complication associated with Moyamoya disease, especially as the condition progresses. The fragile collateral vessels that develop in response to arterial narrowing are prone to rupture due to their thin walls and abnormal structure. When these vessels rupture, they cause bleeding into the brain tissue, leading to hemorrhagic stroke. Symptoms may include sudden severe headache, vomiting, altered consciousness, or neurological deficits. Hemorrhages can be devastating, often resulting in long-term disability or death. The risk of hemorrhage tends to increase with age and disease progression, making vigilant monitoring vital.

Seizures can range from mild to severe and may require ongoing anticonvulsant therapy. The presence of seizures indicates underlying brain injury and highlights the importance of comprehensive neurological care.

The progressive nature of the disease demands a multidisciplinary approach that includes medical management, surgical revascularization procedures, and vigilant monitoring to prevent or minimize these adverse outcomes.
