This condition involves bleeding that accumulates between the dura mater—the outermost membrane covering the brain—and the arachnoid layer, which lies beneath it. The blood collection typically results from torn veins bridging the brain’s surface and the dural sinuses, often caused by rapid acceleration or deceleration movements, such as in car accidents, falls, or sports injuries.

Subdural hematomas can be classified based on their onset and progression. Acute subdural hematomas develop rapidly within 72 hours of injury and often present with sudden neurological deterioration. Subacute hematomas may manifest over days to weeks, while chronic subdural hematomas develop slowly, sometimes over weeks or even months, particularly in elderly individuals or those on blood-thinning medications. The clinical presentation can vary widely, from mild headaches and confusion to severe neurological deficits, including weakness, seizures, or loss of consciousness.

Diagnosis of a subdural hematoma involves neuroimaging techniques, primarily computed tomography (CT) scans, which can quickly reveal the presence of a blood collection beneath the dura. Magnetic resonance imaging (MRI) may also be utilized for more detailed assessment, especially in subacute or chronic cases where the blood may appear differently on imaging. Prompt diagnosis is crucial because the expanding hematoma can increase intracranial pressure, compressing brain tissue and leading to potentially life-threatening complications.

Treatment strategies depend on the size and severity of the hematoma, as well as the patient’s neurological status. Small, asymptomatic hematomas may be managed conservatively with close monitoring, bed rest, and medical management to control symptoms and intracranial pressure. However, larger or symptomatic hematomas often require surgical intervention. Craniotomy or burr hole drainage allows neurosurgeons to

remove the accumulated blood, relieve pressure, and prevent further brain injury. Postoperative care involves monitoring for rebleeding, managing intracranial pressure, and addressing any neurological deficits.

Recovery from a subdural hematoma varies among individuals and depends on factors such as the size of the bleed, the promptness of treatment, and the patient’s overall health. Some may recover fully, while others might experience lasting cognitive or motor impairments. Rehabilitation involving physical therapy, occupational therapy, or speech therapy can be essential for optimizing recovery.

Prevention of closed head injuries leading to subdural hematomas includes wearing seat belts, helmets during sports, and implementing fall prevention strategies, especially for the elderly. Additionally, managing anticoagulant medications carefully can reduce bleeding risks in vulnerable populations.

Understanding the nature of subdural hematomas is vital for early recognition and treatment, which significantly impacts outcomes. As with any head injury, seeking immediate medical attention after trauma is essential to minimize long-term effects and improve prognosis.
