Clival chordoma is a rare, slow-growing tumor that originates from remnants of the notochord, a structure present during fetal development. Positioned at the skull base, specifically near the clivus—a part of the cranial bone just in front of the brainstem—this tumor can pose significant challenges due to its location and potential to affect vital structures. While the exact cause remains unknown, its growth pattern and proximity to critical neurovascular structures necessitate careful diagnosis and management.
Symptoms of clival chordoma often develop gradually, making early detection difficult. Because of its location, the tumor can impinge on the brainstem, cranial nerves, and the spinal cord, leading to a variety of neurological symptoms. Patients may initially notice persistent headaches, especially at the back of the head, which may worsen over time. Double vision or visual disturbances can occur if the tumor presses on the optic nerves or chiasm. Difficulty swallowing or speaking may result from involvement of cranial nerves IX and X, which control these functions. Additionally, symptoms like dizziness, balance problems, or weakness in limbs can emerge if the tumor compresses the brainstem or cervical spinal cord.
As the tumor enlarges, it might also cause hormonal imbalances by affecting nearby pituitary structures, leading to symptoms such as fatigue, unexplained weight changes, or changes in menstrual cycles. Due to its subtle initial presentation, diagnosis often involves neuroimaging techniques like MRI and CT scans, which reveal the tumor’s size, extent, and relationship to surrounding tissues. A biopsy may be performed to confirm the diagnosis histologically, typically revealing a chordoma characterized by physaliphorous (bubble-like) cells.
Treatment for clival chordoma is complex and requires a multidisciplinary approach. The primary goal is complete surgical removal, which can be challenging given the tumor’s proximity to the brainstem, cranial nerves, and major blood vessels. Neurosurgeons often utilize advanced surgical techniques, including endoscopic or open approaches, to access and excise the tumor as thoroughly as possible. Complete resection, when feasible, significantly improves prognosis, but due to the tumor’s infiltrative nature, residual cells are common, necessitating additional therapies.
Radiation therapy plays a vital role in managing chordomas. Conventional radiotherapy may be used post-surgery to target remaining tumor cells, but more advanced methods like proton beam therapy are preferred because they deliver high doses of radiation precisely to the tumor while sparing surrounding healthy tissue. In some cases, stereotactic radiosurgery may be employed for smaller residual tumors.
Chemotherapy has limited effectiveness against chordomas but may be considered in specific situations, especially if the tumor recurs or progresses despite other treatments. Emerging targeted therapies and clinical trials are ongoing to explore more effective systemic treatments.
Long-term care and follow-up are essential since chordomas have a tendency to recur even after initial successful treatment. Regular imaging scans help monitor for recurrence, and supportive therapies such as physical, occupational, and speech therapy can help manage neurological deficits. Additionally, addressing hormonal imbalances or other secondary effects forms part of comprehensive care.
Living with a clival chordoma poses significant challenges, but advances in surgical techniques, radiation therapy, and supportive care continue to improve outcomes. Early diagnosis and a tailored treatment approach are crucial for maximizing quality of life and survival.
