CAN PARASITES CAUSE GRANULOCYTOSIS Parasites are organisms that live on or inside a host, deriving nutrients at the host’s expense. They are known to cause a range of health issues, from mild discomfort to severe disease. While many are familiar with parasitic infections causing gastrointestinal symptoms or skin conditions, their impact on the immune system can sometimes be more complex, influencing blood cell counts and immune responses.

Granulocytosis refers to an elevated level of granulocytes, a type of white blood cell that includes neutrophils, eosinophils, and basophils. This condition often signals an ongoing immune response to infection, inflammation, or other stimuli. Typically, bacterial infections, inflammation, stress, or leukemic processes are associated with granulocytosis. But could parasites also be a contributing factor?

The relationship between parasites and granulocytosis is indeed noteworthy, especially in the context of certain parasitic infections that stimulate the immune system vigorously. For example, helminth infections—caused by worms such as hookworms, schistosomes, or filarial worms—are well-documented to induce eosinophilia, a specific type of granulocytosis predominantly involving eosinophils. Eosinophils are particularly active against multicellular parasites and play a vital role in immune defense. When the body detects parasitic antigens, it releases cytokines and chemokines that promote eosinophil proliferation and recruitment, leading to elevated eosinophil counts in the blood.

In addition to eosinophilia, some parasitic infections can cause a general increase in granulocytes, especially during active infection or tissue invasion. For instance, schistosomiasis, a disease caused by parasitic worms residing in blood vessels, often leads to eosinophilia due to the immune system’s response to the parasite’s eggs and adult worms. Similarly, strongyloidiasis (caused by Strongyloides stercoralis) can sometimes provoke eosinophil elevation, especially during the early stages of infection or during hyperinfection. In clinical practice, recognizing the potential for parasitic infections to cause granulocytosis is crucial, especially in patients from endemic areas or with a history of travel to regions where parasitic diseases are prevalent.
