Initial symptoms of amyloidosis vary depending on the organs affected but often include fatigue, weight loss, swelling, and shortness of breath. For example, cardiac amyloidosis may present with unexplained heart failure symptoms, while renal involvement can manifest as swelling in the legs or proteinuria. Nerve involvement might cause numbness or tingling, and gastrointestinal symptoms can include diarrhea or malabsorption. Because these signs are nonspecific, healthcare providers often face challenges in early detection.

Early diagnosis relies on a high index of suspicion, especially in patients with unexplained organ dysfunction. Blood and urine tests are the first steps, looking for abnormal proteins such as monoclonal light chains. Serum free light chain assays and immunofixation electrophoresis can help identify a systemic amyloid process. Once blood tests suggest amyloidosis, tissue biopsy remains the gold standard for confirmation. Biopsies of affected tissues like the abdominal fat pad, rectal mucosa, or organ-specific tissues can reveal amyloid deposits under microscopy, often with special staining techniques such as Congo red, which exhibits apple-green birefringence under polarized light.

Treatment protocols for amyloidosis are tailored to the type and extent of the disease, but early intervention is vital. The mainstay of treatment aims to reduce or halt the production of amyloidogenic proteins. For AL amyloidosis, which is associated with plasma cell dyscrasias, therapies similar to multiple myeloma are employed, including chemotherapy, immunotherapy agents like dar

atumumab, and autologous stem cell transplantation in suitable candidates. In hereditary forms, managing the genetic defect or reducing amyloid precursor protein synthesis is essential. For AA amyloidosis caused by chronic inflammatory conditions, controlling the underlying inflammation with disease-modifying agents is critical.

Supportive care also plays a significant role in managing symptoms and preventing organ failure. This might include diuretics for heart or kidney involvement, pacemakers for arrhythmias, and nutritional support. Multidisciplinary teams—including cardiologists, nephrologists, neurologists, and hematologists—are often involved in comprehensive management.

Research into targeted therapies and novel agents continues to evolve, offering hope for more effective treatments in the future. Early diagnosis and intervention remain the cornerstones in improving survival and quality of life for amyloidosis patients. Vigilance, prompt testing, and personalized treatment strategies are key to managing this challenging disease effectively.
