Amyotrophic lateral sclerosis (ALS), also known as Lou Gehrig’s disease, is a progressive neurodegenerative disorder that affects nerve cells in the brain and spinal cord. As the disease advances, it leads to muscle weakness, paralysis, and ultimately, respiratory failure. While there is currently no cure for ALS, effective management strategies can significantly improve quality of life, ease symptoms, and prolong survival. These strategies encompass a multidisciplinary approach, addressing physical, nutritional, respiratory, and psychological needs.

One of the foundational aspects of ALS care is symptomatic management. Nutritional support is vital as ALS often impairs swallowing, increasing the risk of aspiration and weight loss. Speech and language therapists assess swallowing function regularly. When swallowing becomes difficult, interventions such as dietary modifications or the use of thickened liquids are recommended. In advanced stages, a feeding tube (percutaneous endoscopic gastrostomy or PEG) may be necessary to ensure adequate nutrition and hydration, reducing the risk of dehydration and malnutrition.

Respiratory management is another key component of ALS care. As respiratory muscles weaken, patients often experience shortness of breath, decreased cough efficiency, and increased risk of respiratory infections. Non-invasive ventilation (NIV) is widely used to support breath

ing, particularly during sleep, and can significantly improve quality of life and survival. In more advanced cases, invasive ventilation through tracheostomy may be considered. Regular pulmonary function tests help monitor respiratory decline and guide timely intervention.

Psychological and emotional support are essential, given the profound impact of ALS on mental health and well-being. Patients may experience depression, anxiety, or feelings of social isolation. Counseling, support groups, and mental health services provide crucial emotional backing. Additionally, advance care planning discussions allow patients to express their wishes regarding life-sustaining treatments and care preferences early in the disease course.

Emerging therapies and clinical trials offer hope for future treatments. While current management remains supportive, ongoing research aims to slow disease progression and discover potential disease-modifying drugs. In the meantime, a comprehensive, multidisciplinary approach remains the cornerstone of ALS management, addressing the complex and evolving needs of patients.

In conclusion, managing ALS requires an integrated approach that combines symptom control, nutritional and respiratory support, and psychological care. Collaboration among neurologists, physiotherapists, speech therapists, respiratory therapists, and mental health professionals ensures that patients receive personalized care tailored to their disease stage and individual preferences. Although ALS remains a challenging condition, strategic management can enhance comfort, preserve function, and improve quality of life for those affected.
