Moyamoya disease is a rare, progressive cerebrovascular disorder characterized by the narrowing or occlusion of arteries at the base of the brain, particularly the internal carotid arteries and their branches. This constriction leads to the development of a network of tiny, fragile blood vessels that attempt to compensate for reduced blood flow. The name “Moyamoya” is derived from a Japanese term meaning “puff of smoke,” which describes the appearance of these abnormal vessels seen in imaging studies. Diagnosing this condition accurately and early is crucial to prevent stroke and neurological deficits.

The process of diagnosing Moyamoya disease begins with a detailed clinical history and neurological examination. Patients often present with symptoms such as transient ischemic attacks (TIAs), strokes, seizures, or headaches. Children may exhibit developmental delays or weakness, while adults might experience sudden numbness or weakness on one side of the body. Recognizing these signs can raise suspicion of a cerebrovascular issue but is not specific enough for diagnosis.

Imaging studies are the cornerstone of diagnosing Moyamoya disease. The initial step usually involves non-invasive modalities such as magnetic resonance imaging (MRI) and magnetic resonance angiography (MRA). MRI provides detailed images of brain tissue, revealing areas of infarction or hemorrhage that suggest ischemic events. MRA allows visualization of the blood vessels, showing the characteristic narrowing and the network of abnormal collateral vessels. However, MRA might sometimes lack the resolution needed for definitive diagnosis.

To obtain a clearer picture of cerebral vasculature, digital subtraction angiography (DSA) remains the gold standard. DSA involves inserting a catheter into the blood vessels and injecting contrast dye to produce detailed images of the cerebral arteries. This technique vividly displays the stenotic segments of the internal carotid arteries and the characteristic “puff of smoke” appearance created by the tangled network of tiny vessels. DSA not only confirms the diagnosis but also helps assess the extent and severity of the disease, guiding potential treatment options.

In some cases, other diagnostic tools such as computed tomography angiography (CTA) are used, especially when DSA is contraindicated or unavailable. CTA offers rapid imaging of cerebral vessels and can demonstrate characteristic features of Moyamoya. Additionally, laboratory tests may be performed to rule out other causes of vasculopathy, such as inflammatory or autoimmune conditions, but they do not confirm the diagnosis of Moyamoya.

The diagnosis of Moyamoya disease requires the exclusion of other conditions that cause similar vascular changes, such as atherosclerosis, vasculitis, or genetic syndromes. Once confirmed, early recognition allows for timely management, which may include medical therapy to prevent strokes and surgical interventions like revascularization procedures to restore adequate blood flow to the brain.

In summary, diagnosing Moyamoya disease involves a combination of clinical suspicion, neuroimaging techniques, and invasive angiography. Understanding these diagnostic steps is essential for clinicians to identify the disease early and improve patient outcomes.
