Laboratory tests alone are insufficient for diagnosing LCH but can assist in evaluating systemic involvement and ruling out differential diagnoses. Blood tests may reveal anemia, elevated inflammatory markers, or abnormalities in liver function tests, especially if internal organs are affected. However, these findings are nonspecific and cannot confirm LCH.
The definitive diagnosis hinges on histopathological examination. A tissue biopsy from an affected lesion remains the gold standard. The biopsy specimen is examined under a microscope to identify characteristic features, including the presence of Langerhans cells. These cells are typically large with a characteristic grooved or indented nucleus and abundant cytoplasm. Immunohistochemistry staining is crucial, with Langerhans cells expressing markers such as CD1a, Langerin (CD207), and S-100 protein. The presence of Birbeck granules, rod-shaped organelles visible under electron microscopy, provides further confirmation but is less commonly performed due to technological requirements.
In some cases, molecular studies may be employed, especially to detect mutations such as the BRAF V600E mutation, which has implications for targeted therapies. However, molecular testing is supplementary and not essential for diagnosis.
