Ischemic and valvular heart disease Ischemic and valvular heart diseases are two of the most common and impactful cardiac conditions affecting millions worldwide. Although they involve different parts of the heart and distinct pathological processes, both can lead to significant morbidity and mortality if not properly diagnosed and managed.

Ischemic heart disease, also known as coronary artery disease, primarily results from the narrowing or blockage of the coronary arteries. These arteries supply oxygen-rich blood to the heart muscle itself. Over time, factors such as atherosclerosis—a buildup of fatty deposits, cholesterol, and other substances—cause the arteries to harden and narrow. This reduced blood flow can lead to chest pain (angina), shortness of breath, and in severe cases, a heart attack (myocardial infarction). The risk factors for ischemic heart disease are well-established and include high blood pressure, high cholesterol levels, smoking, diabetes, obesity, sedentary lifestyle, and family history.

Lifestyle modifications such as healthy diet, regular exercise, smoking cessation, and weight management are foundational. Pharmacological treatments include antiplatelet agents like aspirin, statins to lower cholesterol, beta-blockers, nitrates, and ACE inhibitors. In cases where significant blockages are identified, invasive procedures like coronary angioplasty with stent placement or coronary artery bypass grafting (CABG) may be necessary to restore adequate blood flow.

Ischemic and valvular heart disease Valvular heart disease involves damage or defects in one or more of the heart’s four valves: the mitral, tricuspid, aortic, and pulmonary valves. These valves regulate blood flow direction within the heart, ensuring unidirectional flow during the cardiac cycle. Valvular disorders include stenosis (narrowing of the valve opening) and regurgitation (leakage of blood backward). Common causes include congenital abnormalities, rheumatic fever, degenerative changes, infections like endocarditis, and calcium deposits.

Symptoms of valvular heart disease depend on the severity and specific valve involved. Patients may experience fatigue, shortness of breath, swelling of the ankles or abdomen, palpitations, and chest discomfort. Chronic valvular disease can lead to heart failure if left untreated, as the heart struggles to maintain efficient circulation.

Diagnosis involves a thorough clinical examination complemented by echocardiography, which provides detailed images of valve structure and function. Additional tests such as cardiac MRI, CT angiography, and cardiac catheterization may be employed for comprehensive assessment.

Treatment of valvular heart disease varies based on the severity and symptoms. Mild cases might only require regular monitoring and medical therapy to manage symptoms and prevent complications. In more advanced cases, surgical intervention becomes necessary.
