Hemophilia is a rare inherited bleeding disorder characterized by the deficiency or dysfunction of specific clotting factors, most commonly factor VIII or IX. This condition impairs the blood’s ability to form stable clots, leading to prolonged bleeding episodes that can be spontaneous or triggered by injuries or surgeries. Early and accurate diagnosis of hemophilia is crucial to managing the condition effectively, preventing complications, and improving quality of life.

The diagnostic process begins with a thorough medical history and physical examination. Healthcare providers inquire about any bleeding episodes, their frequency, severity, and triggers. Family history is also assessed because hemophilia is inherited in an X-linked recessive pattern, predominantly affecting males, while females are usually carriers. A history of easy bruising, spontaneous bleeding, bleeding after minor injuries, or bleeding into joints and muscles can raise suspicion.

Laboratory testing forms the cornerstone of hemophilia diagnosis. Initial screening tests include the activated partial thromboplastin time (aPTT), which measures the efficiency of the intrinsic and common coagulation pathways. In hemophilia, aPTT is typically prolonged due to the deficiency of clotting factors VIII or IX. However, these screening tests are not specific and cannot distinguish between different bleeding disorders.

Confirmatory testing involves specific clotting factor assays. These tests measure the activity levels of factor VIII and IX in the blood. In individuals with hemophilia A, factor VIII activity is reduced, often below 25% of normal levels, with severe cases having less than 1%. Hemophilia B is characterized by low factor IX activity. The severity classification—mild, moderate, or severe—is based on the percentage of clotting factor activity, guiding treatment strategies and prognosis.

Additional tests may include mixing studies, which help differentiate between factor deficiencies and the presence of inhibitors—antibodies that neutralize clotting factors. In a mixing study, patient plasma is mixed with normal plasma; correction of the prolonged clotting time indicates a deficiency, whereas lack of correction suggests inhibitors. Genetic testing can also be employed to identify specific mutations in the F8 or F9 genes, especially useful in carrier detection, prenatal diagnosis, and counseling.

It is important to distinguish hemophilia from other bleeding disorders such as platelet function defects or von Willebrand disease, which may require different management approaches. Sometimes, repeated laboratory assessments are necessary to confirm the diagnosis, especially in mild cases where factor levels may fluctuate or be borderline.

In summary, diagnosing hemophilia involves a combination of detailed clinical evaluation, coagulation screening tests like aPTT, specific clotting factor assays, and sometimes genetic analysis. Early diagnosis not only enables timely treatment to prevent serious bleeding episodes but also provides vital information for genetic counseling and family planning.
