Hemophilia is a rare genetic disorder that impairs the blood’s ability to clot properly, leading to excessive bleeding even after minor injuries. Recognizing and diagnosing hemophilia early is crucial to managing the condition effectively and preventing serious complications. The process of diagnosing hemophilia involves a combination of clinical evaluation, detailed medical history, and specific laboratory tests.

Typically, hemophilia is inherited in an X-linked recessive pattern, primarily affecting males, although females can be carriers and occasionally show symptoms. Family history plays a significant role in raising suspicion for the disorder, especially if relatives have experienced frequent bleeding episodes or have been diagnosed with hemophilia. Children presenting with unexplained bleeding, bruising, or bleeding into joints and muscles should prompt further investigation.

The initial step in diagnosing hemophilia is a thorough medical history and physical examination. Healthcare providers ask about the frequency, severity, and triggers of bleeding episodes, as well as any family history of bleeding disorders. The physical exam may reveal signs such as bruises, swelling, or joint deformities resulting from repeated bleeding episodes.

Laboratory testing is the cornerstone of confirming hemophilia diagnosis. The first line of testing involves measuring the activated partial thromboplastin time (aPTT), which assesses the intrinsic pathway of blood coagulation. In individuals with hemophilia, especially severe forms, the aPTT is typically prolonged, indicating a delay in clot formation.

However, a prolonged aPTT alone is not specific for hemophilia and can be caused by other clotting factor deficiencies or inhibitors. Therefore, specific clotting factor assays are conducted to measure the activity levels of factor VIII (for hemophilia A) or factor IX (for hemophilia B). These tests quantify the percentage of normal activity; levels below 1% indicate severe hemophilia, 1-5% suggest moderate, and levels above 5% but below normal range are considered mild.

Additional tests, such as mixing studies, can help determine whether the prolonged clotting time is due to a factor deficiency or the presence of inhibitors that interfere with clotting factors. Genetic testing may also be performed to identify mutations in the F8 or F9 genes, especially in cases where the diagnosis is uncertain or for carrier screening purposes.

It’s important to note that newborns and infants may undergo initial screening with cord blood tests if there is a known family history. In some instances, diagnosis may be delayed until bleeding symptoms manifest. Early diagnosis allows for prompt initiation of treatment strategies, such as factor replacement therapy, and helps prevent joint damage and other complications associated with repeated bleeding episodes.
