Guide to ALS clinical features

Amyotrophic lateral sclerosis (ALS), also known as Lou Gehrig’s disease, is a progressive neurodegenerative disorder that affects nerve cells in the brain and spinal cord. Recognizing its clinical features is crucial for early diagnosis and management, which can significantly impact quality of life and disease progression. ALS primarily targets the motor neurons, which are responsible for controlling voluntary muscle movements. As these neurons degenerate and die, the brain loses its ability to initiate and control muscle movement, leading to a wide array of symptoms.

One of the earliest signs of ALS is muscle weakness, often starting subtly in one part of the body. Patients may notice difficulty with fine motor tasks such as buttoning a shirt, writing, or handling utensils. This weakness typically begins asymmetrically, affecting one limb more than the other, and gradually spreads to other parts of the body. As the disease advances, muscle wasting or atrophy becomes evident, especially in the hands, arms, or legs, due to the loss of muscle bulk.

Spasticity and stiffness are other hallmark features, resulting from upper motor neuron involvement. Patients might experience exaggerated reflexes, such as hyperreflexia, or a spastic gait. These signs reflect the disruption of the descending motor pathways that normally exert inhibitory control over reflexes. The coexistence of upper and lower motor neuron signs is characteristic of ALS and helps distinguish it from other neurological conditions.

As ALS progresses, bulbar symptoms often appear. These involve muscles responsible for speech, swallowing, and breathing. Patients may develop dysarthria, characterized by slurred or nasal speech, and dysphagia, leading to difficulty swallowing and increased risk of aspiration pneumonia. Voice changes, such as hoarseness, can also be observed. Bulbar involvement significantly impacts communication and nutrition, contributing to morbidity.

The respiratory system is eventually compromised as the diaphragm and intercostal muscles weaken, leading to respiratory failure, which is the most common cause of death in ALS patients. Early signs of respiratory compromise include shortness of breath, especially when lying flat, and decreased cough strength. Monitoring respiratory function is a vital component of ALS management.

Cognition and behavior are generally preserved in ALS, although a subset of patients may develop frontotemporal dementia, which affects personality, judgment, and language. Such neuropsychiatric changes can complicate disease management but are not universal.

In summary, the clinical features of ALS encompass a spectrum of motor symptoms—including asymmetric limb weakness, muscle atrophy, spasticity, and bulbar signs—progressing over time to involve respiratory muscles. Recognizing these features early allows for timely intervention, multidisciplinary support, and planning for disease progression. As no cure exists yet, understanding the clinical presentation is essential for optimizing patient care and improving quality of life.
