Cystic fibrosis (CF) is a hereditary genetic disorder that primarily affects the lungs and digestive system. It is one of the most common life-threatening conditions among Caucasian populations, and early detection is crucial for managing symptoms and improving quality of life. Diagnosing cystic fibrosis in children involves a combination of newborn screening, clinical evaluation, and specialized testing.

The journey to diagnosis often begins shortly after birth, especially in regions where newborn screening programs are implemented. These programs aim to identify CF early, before symptoms become severe. The initial screening typically involves a heel prick blood test that measures levels of immunoreactive trypsinogen (IRT), a enzyme that tends to be elevated in infants with CF. Elevated IRT levels prompt further testing to confirm the diagnosis.

If screening indicates the possibility of cystic fibrosis, the next step is usually a sweat chloride test. This diagnostic test measures the amount of salt (chloride) in a child’s sweat. In CF, the sweat glands produce an unusually high concentration of salt due to faulty CFTR (cystic fibrosis transmembrane conductance regulator) proteins. During the test, a small amount of pilocarpine is applied to the skin to stimulate sweating, which is then collected and analyzed. A sweat chloride level above a certain threshold—generally 60 mmol/L—strongly suggests cystic fibrosis.

Genetic testing also plays a vital role in confirming the diagnosis. CF is caused by mutations in the CFTR gene, and over 2,000 different mutations have been identified. A blood sample or buccal swab can be used to analyze the child’s DNA for common CF mutations. Identifying two disease-causing mutations confirms the diagnosis, especially in cases where sweat chloride results are borderline or inconclusive.

In addition to these primary tests, doctors may consider other evaluations, such as pulmonary function tests, imaging studies like chest X-rays, and stool analysis to assess pancreatic function. These additional assessments help in understanding the severity of the disease and tailoring treatment plans.

Diagnosing CF in children is not just about confirming the presence of the disease but also about understanding its impact on various organs. Early diagnosis enables timely intervention, including airway clearance techniques, nutritional support, enzyme replacement therapy, and medications to manage infections and inflammation. Recent advances have also introduced targeted therapies that address specific genetic mutations, offering hope for improved outcomes.

While the diagnosis process can be complex, ongoing research and improved screening methods continue to enhance early detection. The ultimate goal is to diagnose cystic fibrosis as early as possible, ideally in the neonatal period, to start treatments that can extend life expectancy and improve daily functioning.
