Over time, continued inflammation causes excess serum amyloid A to misfold and deposit in organs such as the kidneys, spleen, liver, and gastrointestinal tract.
Another cause of amyloidosis in children is primary amyloidosis, which is linked to plasma cell dyscrasias. Although more typical in adults, cases have been reported in pediatric populations. In this form, abnormal plasma cells produce excessive amounts of monoclonal light chains, which form amyloid deposits. These deposits can lead to organ dysfunction, especially affecting the kidneys and heart.
Genetic mutations are the most prominent cause of amyloidosis in pediatric cases, emphasizing the importance of family history and genetic counseling. Some inherited forms are associated with specific syndromes, such as hereditary systemic amyloidosis, which can involve multiple organs from an early age. Early recognition of symptoms, such as unexplained organ enlargement, neuropathy, or heart failure, can prompt further genetic testing and tissue biopsy to confirm amyloid deposition.
